Notary Acknowledgment Form

The notary public uses a notary acknowledgment form to verify that the person signing the document in their presence is who they say they are. According to the form, the notary witnessed the signer’s signature, obtained proof of the signer’s identity, and was convinced that the signer was willing to sign the form and had the mental capacity to do so.
